2. Have you had close contact with a person who traveled to high-risk sites or who has COVID-19?
3. Do you have a fever and respiratory symptoms such as cough or shortness of breath?

Patients answering yes to any of the above questions are asked to please call the office so that the doctors and staff can discuss how best to take care of patient needs.
